For Individual
Examples: presentation of academic paper abroad, international training opportunity, international competition, etc.
• Original Copy of Solicitation letter (with duplicate)
• Barangay Certificate (Original and Photocopy) for solicitation purposes
• Identification Card (ID) Photocopy
• Residence Certificate (Cedula) Photocopy
• Programme (for trainings/seminars) or Copy of Invitation Letter
For Youth Organizations, Youth-Serving Organizations, Barangay Captain, Sangguniang Kabataan
Examples: sports league, sports competition, trainings, etc.
• Original Copy of Solicitation letter signed by the Chairperson (with duplicate)
Note: If Barangay, the Transfer must be noted by the Barangay Captain and if Municipal/City, the Transfer must be noted by the Mayor.
• Barangay Certificate (Original and Photocopy) for solicitation purposes
• Identification Card (ID) of the Chairperson Photocopy
• Identification Card (ID) of the Barangay Captain Photocopy
• Residence Certificate (Cedula) of the Chairperson Photocopy
• Residence Certificate (Cedula) of the Barangay Captain Photocopy
