| 1. Letter to the Vice Governor |
| 2. Duly accomplished Application Form for Accreditation Click here to download the application form |
| 3. Board Resolution signifying intention for accreditation for the purpose of representation in the local special body |
| 4. Certificate of Registration, or in the case of IPO’s, CoR must be issued by NCIP |
| 5. List of current officers and members |
| 6. Annual Accomplishment Report duly certified by the board secretary of organization |
| 7. Financial Statement, at the minimum, signed by the executive officers of the organization, also of the immediately preceding year, and indicating therein other information such as the source(s) of funds |
| 8. Vision, Mission, Objectives of the organization |
| 9. Copy of the Minutes of the Annual Meeting of the organization duly certified by the board secretary of organization |
